These Terms of Service (“Terms”) govern your use of the website and online services of Miller Chiropractic Wellness Center (“Miller Chiropractic,” “we,” “our,” or “us”). By visiting our website, filling out a form, scheduling an appointment online, or otherwise interacting with our online services, you agree to these Terms. The website is intended for adults age 18 and over.

By using our website or requesting an appointment, you represent that you are at least 18 years old or are the parent/legal guardian of a minor patient.

We reserve the right to refuse or cancel appointments, suspend access, or terminate online interactions for any misuse of the site or violation of these Terms.


Not for emergencies:
The website, forms, email, and text messaging are not for urgent or emergency medical situations. If you think you are having a medical emergency, call 911 or go to the nearest emergency room.


2. No Medical Advice by Website

Information on this website (including articles, graphics, videos, and any automated messages) is for general educational purposes only and is not a substitute for an in‑person evaluation by a licensed healthcare professional.

Nothing on this site creates a doctor–patient relationship. Diagnosis and treatment decisions are made only after an appropriate clinical evaluation in our office.

4. Appointments, Payment & Insurance

Appointment requests submitted online are requests only until confirmed by our staff.

Fees, copays, and deductibles are due as explained at the time of service.

We may assist in submitting claims to your insurance carrier as a courtesy, but you remain responsible for all charges not covered by insurance.

Any special offers (e.g., free scans, introductory promotions) apply only as described in the specific offer and may be changed or discontinued at any time.

Unless a written agreement or promotion says otherwise, all services rendered are non‑refundable.


5. Privacy & Health Information

Your use of our website and online forms is also governed by:

- Our online Privacy Policy (if posted on the site), and

- Our in‑office Notice of Privacy Practices, which describes how we protect and use your health information in accordance with applicable law (including HIPAA, where applicable).

We encourage you to first contact us to resolve any concerns. If a dispute cannot be resolved informally, you agree that any claim relating to the website or these Terms will be brought in the state or federal courts located in New Jersey, and you consent to their jurisdiction.

To the extent permitted by law, you agree to bring any claims individually and not as part of a class or representative action.
